Occupational Therapy Case Management plays a crucial role in helping individuals improve their functional abilities and overall well-being.
Occupational therapy outcomes refer to the results that clients achieve through intervention. These outcomes are established using assessment tools and
performance measures. Our Occupational Therapy services help individuals by:
-Enhancing an individual’s ability to engage in meaningful daily activities.
-Addressing risk factors and promoting health to prevent future issues.
-Supporting overall physical and mental well-being.
-Improving an individual’s overall satisfaction with life.
-Encouraging active involvement in social, work, and leisure activities.
-Helping patients reach their own driven outcomes, these can be things like confidence, self-
efficacy, pain-reduction, or more.
-Integrating outcomes into practice and creating plans for sustainability.
Remember that occupational therapy aims to enhance individuals’ quality of life, promote independence, and facilitate meaningful participation in daily activities.
Peer-to-Peer Support
Our Peer-to-Peer Support Program at Pathway To Hope pairs up an individual struggling with mental illness with a trained peer who has walked a similar journey to provide them with much needed support from a place of understanding. Trained peers can provide a supportive environment, help map out goals, identify urgent needs, and assist them in meeting those urgent needs.
